  • Versione Italiana: È indubbio che nell’uomo l’invecchiamento sia correlato con il progressivo decremento dell’attività di tutti i sistemi organici e di tutte le funzioni, da quella muscolare conseguente alla sarcopenia alle funzioni cognitive superiori conseguenti alla riduzione del numero di neuroni. Anche l’apparato vestibolare, responsabile della sensazione di accelerazione angolare e gravito- inerziale e del controllo dell’orientamento della testa rispetto alla gravità, subisce questo fenomeno ed in Letteratura sono molti gli Autori che lo hanno descritto. La valutazione funzionale del sistema dell’equilibrio nel paziente anziano è direttamente l’espressione di alterazioni periferiche e compensi funzionali centrali che possiamo osservare complessivamente attraverso le variazioni dei test strumentali nella pratica ambulatoriale. Nelle pagine che seguono verranno analizzati nell’ordine i risultati ottenuti con la prova calorica bitermica binaurale alternata secondo Fitzgerald e Hallpike, i VEMPs, il test vibratorio, i movimenti saccadici, lo smooth pursuit e l’Head Impulse test digitale (Video Head Impulse test) e funzionale (Functional Head Impulse test).

    English Version: There is no doubt that aging is related to the progressive decrease in the activity of all the organs and functions, from the muscular consequence of sarcopenia to higher cognitive functions resulting in the reduction of the number of neurons. Also the vestibular system, responsible for angular acceleration and gravity-inertial sensation, and the control of head orientation with respect to gravity undergo this phenomenon, and many authors have described it in literature. The functional evaluation of the balance system in the elderly patient is directly the expression of peripheral alterations and central functional compensations that we can observe altogether through the variations in instrumental testing in outpatient practice. In the pages that follow, the results obtained with Fitzgerald and Hallpike alternate binary bithermic caloric test, VEMPs, vibratory test, saccadic movements smooth pursuit and the Head Impulse digital test (Video Head Impulse Test) and functional (Functional Head Impulse Test) will be discussed.
